A discharge partner that clears what clinical teams cannot
Senior Home Solutions serves hospital case managers, discharge planners, and elder law attorneys as a single point of accountability across clinical care, licensed home modification, and estate real estate.
The Problem We Solve
The most common obstruction in a discharge is not clinical, it is structural. A patient is medically ready, yet no safe destination exists: the home has stairs and no care coverage, the family has equity but no liquidity, and the community list is a stack of brochures no one has time to reconcile.
That is a social discharge blockage, and it is our specialty. We are staffed and licensed to act where a case management office cannot: we assess the clinical care plan against the physical home, we perform and warrant licensed structural modifications under MHIC #155639, and we coordinate complex private-duty nursing through our clinical partner Liberty Advanced Nursing.
One call opens the file, and we close the loop. The hospital gets a discharge date it can trust, the attorney gets a transition plan that accounts for the estate, and the family gets one accountable partner instead of four vendors.
Capabilities · What We Bring to the Case
Three capabilities that address the causes of delayed discharge
Capability 01
Breaking social discharge blockages
The asset-rich, cash-poor family is the recurring profile behind the extended stay: a paid-off home worth substantial equity, retirement income that cannot cover private care, and no obvious source of bridge funding. Nobody on the care team is positioned to connect all of those pieces, and the hospital absorbs the delay.
We can. The equity in the home is an asset we can mobilize through a coordinated real estate plan while the clinical and structural bridge is built underneath it: safe destination confirmed, modifications scheduled, care coverage in place under Liberty Advanced Nursing, and the estate timeline set, all in one file.
Capability 02
Preventing 30-day readmissions
Readmission risk is highest in the first weeks after discharge, when the home environment fails the care plan: an unmodified bathroom, an unsupervised medication routine, an older adult whose condition needs skilled supervision. Path 01 exists to close exactly that gap.
We coordinate a safe, high-acuity bridge back to the community through Liberty Advanced Nursing, our clinical partner for complex private-duty care, while the home is simultaneously modified under MHIC #155639. The older adult returns to a home that has been clinically and structurally prepared for them, and the discharge team receives a written record of both.
Capability 03
A single point of accountability
Multi-disciplinary expertise is only useful if someone is accountable for the whole. Senior Home Solutions holds the file across clinical care, licensed structural modifications, and estate liquidation under one roof, led by one principal, Vashon Hargrove, REALTOR® and Associate Broker with Samson Properties.
For the elder law attorney, that means a transition plan that respects the estate plan, a documented modification record, and a real estate execution under a licensed broker. For the facility and the case manager, it means one phone number that answers for the timeline, the care, and any contingencies.
Referral Protocol
How to refer a family
A referral takes a single call or a short form. We accept referrals from case managers, discharge planners, physicians, and attorneys, and we return a written transition plan to the referring professional for the file.
