Services · The Four Paths

Four paths, and we help you choose the right one

Every engagement begins with a conversation. We meet with your loved one and their family, take time to understand the full situation, and review each option together before any plan begins. Each path is explained plainly below, and each one stands on its own.

How It Works

One conversation. We meet with your loved one and their family and listen first: health, home, finances, and what everyone wants. Then we go through each option together and talk it through before any plan begins. No forms pushed across the table, just a conversation.

One honest look at all four paths. Home, community, sale, and care are each considered on their own. We never roll them into one package that claims to handle everything.

One path, planned with you. We recommend the path that fits, explain what it involves, what it costs, and who does what. Then we carry it out with your family from the first call to the settled transition.

Path 01 · Home Safety & Aging in Place

Home Safety & Aging in Place

Home assessments, senior-focused repairs, fall prevention, and accessibility modifications. This path is about making the family home a safe place to stay.

Home safety assessments

A professional home safety assessment under CAPS (Certified Aging-in-Place Specialist) and SHSS (Senior Home Safety Specialist) method: falls risk, bathroom safety, stairs, lighting, medication safety, and emergency access.

Senior-focused repairs & fall prevention

The repairs that make a home safe come first: fixing trip hazards, adding secure handholds, improving lighting, and removing the small risks that cause falls.

Accessibility modifications

Grab bars, ramps, stairlifts, and bathroom retrofits are designed from the assessment and performed under the Maryland Home Improvement Commission contractor license MHIC #155639. Licensed, insured, and documented for the record.

Path 02 · Senior Living Placement

Senior Living Placement

Assisted living, independent living, memory care, and transition assistance. When a community is the right setting, we navigate placement for the family.

Assisted living

Communities are screened for level of care, staffing, and acuity tolerance against your loved one's assessed needs, not against referral fees.

Independent living & memory care

One file across independent living, assisted living, and memory care, so the search moves with the family's needs without restarting.

Transition assistance, free to the family

OHCQ-compliant placement navigation. The community pays for placement; the family never does, and the fee structure is disclosed in writing before any community is introduced.

Path 03 · Senior Real Estate & Home Sales

Senior Real Estate & Home Sales

Home sale planning, property preparation, downsizing, and relocation coordination. When the family home sells, the sale is planned and managed.

Home sale planning

A clear plan for if and when the home sells: timing, pricing, and how the proceeds fund the transition.

Property preparation & downsizing

The home is prepared to sell well, and the household is downsized with a written record: keepsakes to family, the rest donated, sold, or disposed through the most appropriate channel.

Relocation coordination

The move itself is managed end to end against one transition calendar, and the sale is executed through The Hargrove Group of Samson Properties under Maryland, Virginia, or DC licensure.

Path 04 · Home Care & Nursing Solutions

Home Care & Nursing Solutions

Assistance identifying appropriate in-home support, from companion services to skilled nursing. Families get the care that fits, never more than they need and never less.

Identifying the right support

We help your family understand what level of in-home support fits: companion services, personal care, or skilled nursing, matched to the assessed care plan.

From companion care to skilled nursing

The full range of in-home support is reviewed fairly, so the family chooses the level of care that is actually appropriate, documented, and adjustable as needs change.

Complex care through Liberty Advanced Nursing

When the plan requires skilled, high-acuity care at home, it is coordinated through our clinical partner, Liberty Advanced Nursing, so the care keeps the stay safe through the night and the week.

The Process · What to Expect

It begins with a conversation

We begin by meeting with your loved one and your family, taking time to understand the full situation, and together you discover which path is right. Every option is considered carefully, and one honest recommendation is made with the family. A more detailed description of what to expect, from the conversation to the chosen path, is on the Process page.

First

The conversation

We meet with your loved one and your family and review the full picture: health, home, finances, and what everyone wants. Then we discuss each option with you before any plan begins.

Then

Discovery

Home, community, sale, and care are each reviewed on their own, so the choice the family makes is a real one.

Finally

The chosen path

We recommend the path that fits, explain it clearly, and plan it with the family step by step.
