Path 04 · Home Care & Nursing Solutions
Home Care & Nursing Solutions
We help you explore private-duty and other home care options, including advanced nursing services when appropriate, so you or your loved one can remain safe, comfortable, and independent at home.
What This Path Is
The first question for many families is whether their loved one can be cared for at home, and what that would take. We help families understand what level of in-home support fits, from companion services to skilled nursing, matched to the assessed care plan.
Complex, high-acuity care is never improvised. It is coordinated through our clinical partner, Liberty Advanced Nursing, under the direction of its Doctor of Nursing, Dr. Keisha Rollins-Monroe. The care keeps your loved one comfortable in their home, which allows them to stay home.

Learn moreHow Care Works
The right level of care, matched to the plan
The full range of in-home support is reviewed fairly, so the family chooses the level of care that is actually appropriate, documented, and adjustable as needs change. When the plan requires skilled, high-acuity care at home, it is coordinated through Liberty Advanced Nursing, so the care is reliable around the clock and through the week.
- Companion to skilled. The whole range is reviewed, never more than needed and never less.
- Clinical partner led. Complex care is directed by Liberty Advanced Nursing's Doctor of Nursing.
- Adjustable as needs change. The plan is reviewed and updated, and the family is kept informed.
