Skilled nursing at home

When the care plan requires skilled, high-acuity care at home, it is coordinated through our clinical partner, Liberty Advanced Nursing, under the direction of its Doctor of Nursing, Dr. Keisha Rollins-Monroe.

What Skilled Nursing Is

Skilled nursing is licensed nursing care delivered in the home for complex, high-acuity needs: wound care, medication management, IV therapy, post-surgical recovery, ventilator or monitoring support, and the care plans that a companion or personal care aide cannot safely carry.

It is the level of care that makes a hospital discharge to home possible for the most complex patients, and it is the level of care that keeps a loved one at home through the night and the week when their condition demands it.

How It Works

Assessment, care plan, and a team that keeps you informed

The assessment

If it is determined that an assessment is needed, we connect you with one of our professional, highly skilled clinicians to conduct an assessment.

The care plan

From that point, a care plan is created, written down, and shared with the family and the referring professionals.

Kept up to date

We keep you updated on your loved one's care and any adjustments needed, so the family is never the last to know.

Why Liberty Advanced Nursing

Complex care is never improvised

Complex private-duty care is coordinated through our clinical partner, Liberty Advanced Nursing, under the direction of its Doctor of Nursing, Dr. Keisha Rollins-Monroe. The care keeps your loved one comfortable in their home, which allows them to stay home.

  • Clinically directed. Care standards are set under a Doctor of Nursing.
  • High-acuity capable. The bridge home for the most complex discharges.
  • Stays home. This care keeps them comfortable in their home, which allows them to stay home.

Who This Fits

Skilled nursing fits when the care plan is complex enough that a licensed nurse must be in the home, and when the goal is to recover, or live, at home rather than in a facility.

  • After a complex hospital discharge
  • Wound care, IV therapy, or medication management
  • Post-surgical recovery at home
  • The family wants skilled care without a facility move
