Transition timeline planner
A week-by-week planner for any senior transition, from the decision through the settled move. Use it with the family, share it with us, and keep every task on one calendar.
The Timeline
Twelve weeks, four phases
Most transitions fit a twelve-week rhythm when there is no crisis forcing a faster one. A hospital discharge compresses the same phases into days; the planner still works, just faster.
Weeks 1 to 3 · The Decision
- Hold the family conversation. Sit down together, listen, and put every option on the table.
- Take the needs assessment. It frames the situation, care needs, home safety, timing, and assets.
- Review the four paths with us and choose the one that fits.
- If a community is in play, start the placement search and the comparison guide.
Weeks 4 to 6 · The Assessment
- Schedule the home safety assessment if staying home is on the table.
- If care is the question, arrange the clinical assessment through Liberty Advanced Nursing.
- If the home may sell, get the sale plan: timing, pricing, and the proceeds.
- Run the fall-prevention checklist through the home while you wait.
Weeks 7 to 9 · The Plan
- Confirm the chosen path and the written plan: providers, dates, and who pays for what.
- Schedule the licensed modifications or repairs if staying home.
- Shortlist communities and take the tours with the questions checklist.
- Start the downsizing with the room-by-room checklist.
Weeks 10 to 12 · The Move
- Coordinate the move against one calendar: movers, utilities, forwarding, and setup.
- Complete the property preparation and list the home if it is selling.
- Move in, settle in, and confirm the care or community plan is running.
- Check in with us through the settling period.
When Time Is Short
The discharge timeline: days, not weeks
A hospital discharge does not wait for a twelve-week plan. When the timeline is days, the same phases compress: the assessment happens at the bedside, the care plan is built before discharge, and the home is modified or the community secured on a hard date.
- Same-day triage. Active discharges are triaged the same day the call comes in.
- One accountable coordinator. Care, home, and estate are coordinated in one file.
- For professionals. Case managers and discharge planners: see the referral protocol.
