Path 02 · Transition Assistance
Transition assistance
The whole move into a community, coordinated end to end: the search, the tours, the paperwork, the move-in, and the settling in. It is free to the family. The community pays for placement; the family never does.
What This Service Is
Choosing a community is one thing; actually getting there is another. Transition assistance covers the entire move: the shortlist, the tours, the questions to ask, the application, the level-of-care assessment the community requires, the move-in date, and the settling-in period after the door closes.
The service is OHCQ-compliant, and the fee structure is disclosed in writing before any community is introduced. The community pays for placement; the family never does. One file follows the transition from the first tour to the settled move.
How It Works
One file, from first tour to settled move
- 1
Assess
The needs assessment establishes the level of care and the search criteria.
- 2
Shortlist
Communities are screened across Maryland, Virginia, and DC and shortlisted to a real few.
- 3
Tour & choose
We tour with the family, ask the hard questions, and help them choose with confidence.
- 4
Move in
Applications, assessments, and the move itself are coordinated against one calendar.
- 5
Settle in
We check in through the settling period, and the family knows who to call.
Free to the Family
The community pays. The family never does.
Placement services are compensated by the communities we work with, which is why the family pays nothing. That arrangement only works if the family trusts the recommendation, which is why every community is screened against the assessed needs, and why the fee structure is disclosed in writing before any community is introduced.
- No fee to the family. Not for the search, not for the tours, not for the move-in.
- Disclosed in writing. The fee structure is put in writing before any community is introduced.
- OHCQ-compliant. Placement navigation follows Maryland's Office of Health Care Quality standards.
Who This Fits
Transition assistance fits any family moving a loved one into a community, whether the decision took a year or a week. It is especially valuable when the move follows a hospital discharge and the timeline is tight.
- Moving to assisted living, independent living, or memory care
- Following a hospital discharge with a tight timeline
- The family is overwhelmed by brochures and tours
- The move needs to coordinate with a home sale
